Periacetabular osteotomy for dysplastic hip joint – experiences in Helsinki

چکیده
e Bernese rotational acetabular osteotomy (RAO) was developed in 1983 (1) in order to prevent the development of arthrosis in young patients with hip dysplasia, and several reports on satisfactory results have subsequently been published (2–9). e RAO performed through the two most frequently used approaches, the ilioinguinal and Smith-Petersen, requires a broad dissection of the pelvic bones and detaching of muscle insertions (10). A less invasive method which was developed initially for pelvic and acetabular fractures was modifi ed for rotational acetabular osteotomy in 1994 at our department. is two-incision anterior technique includes both an extraperitoneal and an anterolateral exposure (ilioanterior approach) of the pelvic bones allowing an unconstrained approach to the medial acetabular wall leaving the lateral true acetabular area untouchable (11).

Comparison of periacetabular osteotomy and total hip replacement in the same patient: a two- to ten-year follow-up study.
We evaluated 31 patients with bilateral dysplastic hips who had undergone periacetabular osteotomy for early (Tönnis grade 0 or 1) or moderate (Tönnis grade 2) osteoarthritis in one hip and total hip replacement for advanced (Tönnis grade 3) osteoarthritis in the other. At a mean follow-up of 5.5 years (2 to 9) after periacetabular osteotomy and 6.7 years (3 to 10) after total hip replacement, ...
متن کاملEvaluation of Bernese periacetabular osteotomy: prospective studies examining projected load-bearing area, bone density, cartilage thickness and migration.
The typical dysplastic hip joint is characterised by maldirection of the acetabulum and femoral neck, insufficient coverage of the femoral head focally and globally and erosions of the limbus acetabuli (1). An unknown number of persons with hip dysplasia will suffer from pain in hip or groin, decreased hip function and development of osteoarthritis at a young age. The Bernese periacetabular ost...
متن کاملAssessment of adult hip dysplasia and the outcome of surgical treatment.
Hip dysplasia and hip joint deformities in general are recognized as possible precursors of osteoarthritic development. Early and correct identification of hip dysplasia is important in order to offer timely joint preserving treatment. In the contemporary literature, several controversies exist, and some of these were the focus of this doctoral thesis. Categorized into subjects, the major findi...
متن کاملPeriacetabular osteotomy for painful non-paralytic dysplastic hip joints in adults affected by poliomyelitis.
BACKGROUND Few researchers have discussed hip joint dysplasia in adults affected by poliomyelitis. We retrospectively studied the outcomes of hip joint function in poliomyelitic adults who underwent periacetabular osteotomy for the contralateral painful non-paralytic dysplastic hip joints. METHODS Eight female patients with the mean age of 35.9 years underwent periacetabular osteotomy from Ja...
متن کاملPeriacetabular osteotomy reduces the dynamic instability of dysplastic hips.
We compared the dynamic instability of 25 dysplastic hips in 25 patients using triaxial accelerometry before and one year after periacetabular osteotomy. We also evaluated the hips clinically using the Harris hip score and assessed acetabular orientation by radiography before surgery and after one year. The mean overall magnitude of acceleration was significantly reduced from 2.30 m/s(2) (sd 0....
